Job description

TRANSITION SPECIALIST 

Academy - Framingham 

Wayside’s Academy is looking to hire a Transition Specialist, responsible for developing curriculum and structures to ensure that all students will graduate high school with the skills needed to be ready for college and career. The ideal candidate will apply knowledge of work-based learning and an understanding of the unique needs of the Academy population to work collaboratively with agency representatives and community partners to facilitate both on and off-campus learning experiences designed to promote transition skill-building.  

Salary Range: $24.00 - $25.00/hour depending on experience + sign-on bonus (half paid after 6 months of employment and half paid after 1 year of employment/pro-rated for part-time).

What You Will Do: 

  • Assist the Academy Administration Team to develop, implement, and monitor a plan to provide appropriate and effective transition services.
  • Conduct informal and formal transition assessments and/or monitor the administration of transition assessments administered by campus staff to identify strengths, needs, preferences, and interests. 
  • Counsel students regarding transition assessment results and post-secondary goals related to education/training, employment, and independent living if appropriate. 
  • Provide professional development and technical assistance on transition service requirements and the appropriate implementation by campus staff.  
  • Act as the transition contact person for administrators, teachers, parents, and community partners. 
  • Collaborate with staff in the preparation of the IEP and transition services. 
  • Attend IEP Meetings to assist in the development of transition services, as needed. 
  • Works collaboratively with agency representatives to provide information sessions for students and parents, conduct eligibility and application activities, and work to transition students from public school services and support to adult services and support.  
  • Conduct regular evaluations of transition services' effectiveness and compliance. 
  • Organize and schedule student and job coach assignments and transportation to and from work-based learning experiences.  
  • Meet with employers/employees at non-paid, work-based learning sites before placing students to establish a meaningful learning format, assessment process, and collaborative support system. 
  • Record, collect, and analyze performance data on work-based learning sites to make data-driven decisions regarding instruction, re-teaching, support, and site assignment through task analysis, and job-related behaviors. 
  • Attend program functions and parent engagement opportunities as scheduled.

  • Demonstrates the ability to teach, mentor, and coach others (i.e. able to provide constructive feedback to Teachers and aides regarding reading instruction, and provide thoughtful instruction).
  • Demonstrates dependability and persistence (i.e. excellent follow-through on assigned tasks).
  • Possesses the capacity to foster and maintain relationships with community businesses, appropriate work experiences, transition supports, and post-secondary education settings. 
  • Ensure compliance with Wayside Youth and Family Support Network policies and procedures.

MinimumRequirements: 

  • Bachelor's degree in Education.  
  • At least 1+ year experience within an academic setting (preferably specialized environments). 
  • Some experience in work-based learning and/or Career and Technology Education (preferably within the specialized environment and/or working with students receiving special education services). 
  • Transition Specialist Endorsement preferred.

We will train you in:Motivational Interviewing; Courageous Conversations; Attachment, Regulation, and Competency Model (ARC); CPR, and First Aid. Â
