Transition Youth Specialist
Our Transition Youth and Young Adult Services team at Southeast Healthcare Services is seeking a dedicated and qualified professional to provide community-based services to youth ages 14–18. This role is focused on supporting participants as they develop and strengthen independent living skills and employment readiness.
Key Responsibilities:
- Conduct initial and ongoing assessments of participants’ independent living skills, vocational strengths, and interests
- Develop individualized service plans aimed at achieving participants' goals in areas such as education, employment, and daily living
- Provide hands-on support and coaching to assist youth in obtaining and maintaining employment
- Facilitate access to resources and create learning opportunities that promote self-sufficiency
- Collaborate with service providers, families, and systems of care to ensure coordinated support
Ideal Candidate Will Have:
- Experience working with transition-age youth
- Strong understanding of independent living and vocational support services
- Ability to engage young people in a strengths-based and goal-oriented manner
- Excellent organizational and communication skills
- A commitment to empowering young people toward greater independence

Requirements
- Must be willing to transport youth in personal vehicle as needed.
- BA/AA degree required.
