Transition Coordinator
We are seeking a highly organized, collaborative, and student-centered Transition Coordinator to join our team. The ideal candidate will have experience supporting students with disabilities in preparing for life beyond high school, with a strong emphasis on transition planning, communication, collaboration, and community partnerships. This role involves working closely with educators, families, students, and community agencies to develop and implement meaningful transition services that promote independence, employment, and post-secondary success.
Position Overview:
The Transition Coordinator plans, coordinates, delivers, and evaluates transition education and services at both the school and system level in collaboration with educators, families, students, and representatives from community organizations and local businesses. This role focuses on supporting middle and high school students (ages 14-22) with disabilities as they prepare for successful post-graduate adult life.
This position is ideal for a passionate professional dedicated to empowering students with disabilities to achieve meaningful, independent, and fulfilling adult lives through strong transition planning and community collaboration.
Requirements:
Related degree and/or a minimum of 5 years of experience and/or training in the area of work study and transition training for special education students
2. Knowledge of the Individual with Disabilities Education Improvement Act, the American Disability Act, Employment First Initiative and the Transition Services Process
3. Knowledge of vocational/transitional education programs
4. Knowledge of community agencies
5. Knowledge of special education research and scientific research based practices
6. Ability to effectively communicate and work with students of all disabilities, community agencies,
parents, post secondary education institutions, and staff
7. Possess effective written and verbal communication skills
8. Possess knowledge of technology
9. Possess effective active listening skills
10. Possess organizational problem-solving skills
11. Possess professional appearance and disposition
12. Possess leadership skills
