Job description

Overview

The Care Transition Assistant supports the Case Management staff by providing clerical functions for the department's Case Managers and Social Workers.  These tasks include but are not limited to: arranging post discharge services, issuing Medicare Important Message letters and communicating updates to the hospital care team staff

Qualifications

Education:  Requires High School diploma

Experience:  Service in a customer service role preferred; Hospital experience preferred

Additional Skills/Abilities:  Care Transition Assistant must be able to work independently and possess excellent verbal and written communication skills.  It is imperative that the Care Transition Assistant has the ability to function as a team member and appropriately seek supervision as needed. Must demonstrate the ability to organize and prioritize and the ability to interact with all levels of staff in a courteous and professional manner.

Must have computer skills
