JOB SUMMARY:
The Transition Facilitator supports students in the areas of academic achievement, behavior, social/emotional development, attendance, and successful transition to and from the SCOPE Alternative Education Program. This position serves as a liaison among students, families, SCOPE staff, sending schools, and community partners; coordinates student intakes/exits and transition activities; monitors student progress; analyzes student data; and assists with behavioral, academic, and social/emotional supports.
QUALIFICATION REQUIREMENTS:
  • Must hold or be eligible for an educator license in the State of Delaware
  • Minimum of four (4) years of successful experience working with school-age students, preferably in an educational, counseling, behavioral, social work, alternative education, or student support setting.
  • Demonstrated experience working with students who exhibit behavioral, social/emotional, attendance, academic, or other barriers to school success.
  • Experience coordinating meetings, student services, transition plans, or other multi-stakeholder processes preferred.
  • Knowledge of effective transition practices and the ability to support students in successfully returning to their home school or progressing toward postsecondary goals.
  • Demonstrated ability to communicate effectively, both orally and in writing, with a diverse range of students, families, and professionals.
  • Ability to maintain confidentiality and handle sensitive student and family information with professionalism and discretion.
  • Ability to work effectively as part of a multidisciplinary team and collaborate with multiple school districts and outside agencies.
  • Strong organizational, time-management, documentation, and follow-through skills.
  • Ability to travel between SCOPE locations and attend meetings at sending schools and other locations as required.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
  • Review student referrals and coordinate and lead intake/exit meetings with students, families, and sending school representatives.
  • Serve as a liaison among SCOPE, students, families, sending schools, and community partners to support student success.
  • Monitor student academic progress, attendance, behavior, and discipline during placement and, as appropriate, following exit from SCOPE.
  • Analyze student data to identify needs, concerns, trends, and appropriate supports.
  • Serve as a student mentor and provide individual support, guidance, and assistance as needed.
  • Assist with student scheduling, discipline, behavioral interventions, and crisis management.
  • Collaborate with teachers, administrators, families, sending schools, and community agencies to address student needs and promote successful outcomes.
  • Represent SCOPE at student transition and exit meetings at students' home schools and assist with transition planning.
  • Assist students in developing goals and making informed decisions regarding college, career, military, workforce, and other postsecondary opportunities.
  • Facilitate state, district, and other testing programs as assigned.
  • Maintain accurate student records and documentation related to progress, interventions, meetings, and transition activities.
  • Attend required meetings, professional development, and program activities.
  • Other duties as assigned.

WORKING CONDITIONS AND EXPECTATIONS:
This is a 10-month position based on a 188-day annual contract. Employees are expected to work 7.5 hours per day, consistent with the established school and district schedules. Responsibilities may include duties beyond instructional time, such as meetings, professional development, and school-related events. Employees are expected to maintain regular attendance, fulfill all assigned responsibilities, and demonstrate flexibility to meet the evolving needs of students, staff, and the district. This position may require additional work days during the summer months beyond the standard school-year schedule.
