TRANSITION SERVICES TEACHER (GRADES 9-12) (SY2026-27)
Terms of Employment:  Full-time, 10-month
Reports to: Principal, High School

POSITION OVERVIEW: The Transition Coordinator plays a vital role in guiding students with disabilities toward meaningful futures beyond high school, supporting their access to higher education, workforce participation, and self-sufficient living. This position is responsible for ensuring that transition planning is purposeful, compliant with federal and state regulations, and tailored to each student's unique abilities, preferences, and aspirations. The coordinator works in close partnership with students, their families, school personnel, and community-based organizations to design and deliver transition services integrated within the IEP framework.
 

Minimum Qualifications:

  • Must hold a valid, active Delaware Teaching License and Certificate in required content area in accordance with 14 Del. C. 1531.
  • Successful completion of Praxis II exams.
  • Master's degree in Special Education or a related field preferred
  • Minimum of four (4) years of successful experience in special education preferred
  • Thorough understanding of IDEA transition mandates and the IEP development process
  • Proficient in English Language, both written and spoken
  • Experience in an immersion or foreign language program
  • Experience teaching students with diverse needs, specific background working alongside high school students presenting a range of learning differences
  • A global citizenship and collaborative mindset
  • Experience working in a multicultural/multilingual environment
  • Comprehensive knowledge of post-secondary options spanning higher education, career pathways, and vocational or trade programs
  • Demonstrated ability to build productive working relationships with students, families, educators, and external partners
  • Excellent organizational abilities, interpersonal skills, and proficiency in case management
  • Prior experience in service or program coordination is a plus 

Essential Duties and Responsibilities:

  • Lead and support the creation of transition plans within IEPs for secondary students between the ages of 14 and 21
  • Work alongside Special Education Coordinator to uphold adherence to applicable federal, state, and district mandates governing transition services
  • Partner with special education teachers, guidance counselors, and school leadership to ensure transition objectives reflect individual student needs
  • Champion student-driven planning processes encompassing goals related to higher education, career readiness, and daily living independence
  • Link students and their families to relevant community organizations, vocational training opportunities, and post-secondary support systems
  • Plan and facilitate experiential learning opportunities such as job shadows, internships, work-based placements, and career exploration activities
  • Track each student's advancement toward transition benchmarks and modify plans accordingly
  • Ensure thorough and accurate recordkeeping pertaining to transition services, progress, and measurable outcomes
  • Deliver professional development and instructional support to colleagues regarding evidence-based transition practices
  • Attend and contribute to IEP meetings and interdisciplinary team conferences
  • Empower students to build self-advocacy skills, life skills, and readiness for the workplace
  • Contribute to the collection and analysis of data related to post-school results and federal compliance indicators
  • Maintain up-to-date knowledge of evolving legislation, policy, and research-informed practices in transition education
  • Fulfill additional responsibilities as directed by Principal, ELT, and administration
