Job Posting: Transition Program Coordinator (Ages 18-22)

Our 18-22 Transition Program is dedicated to providing community-based vocational training, functional life skills instruction, and person-centered planning, empowering students to thrive in adulthood.

Position Summary

We are seeking a passionate and proactive Transition Coordinator to lead our 18-22 Transition Program. This individual will oversee the development of individualized transition plans, secure community-based vocational opportunities, and supervise job coaches to ensure students develop necessary skills for employment and independent living.

Key Responsibilities

  • Vocational & Employment Development: Identify, develop, and maintain community-based work sites (internships, paid employment) for students.
  • Job Coach Supervision: Supervise, train, and evaluate job coaches to ensure consistent, high-quality support to students in vocational settings.
  • Transition Planning & IEPs: Collaborate with IEP teams to develop comprehensive, measurable transition goals focused on employment, education, and adult living.
  • Case Management & Instruction: Oversee a caseload of students, providing direct instruction in life skills, career readiness, community participation, and self-advocacy.
  • Agency Collaboration: Act as the liaison between the school, families, community partners, and state agencies (e.g., DDS, BRS, Vocational Rehabilitation) to ensure seamless, holistic planning.
  • Data Collection & Assessment: Administer transition assessments to determine student interests, aptitudes, and needed supports; maintain accurate progress documentation.

Qualifications

  • Education: Bachelor's Degree in Special Education or related field required; Master's Degree preferred.
  • Licensure: Valid State Special Education Certification 
  • Experience: Minimum of 3+ years of experience working with transition-age students (16-22) with disabilities preferred, however not required.
  • Skills: Demonstrated ability to network with community employers, supervise staff, and develop individualized vocational plans.
