Description
BRONX APARTMENT PROGRAM SENIOR CASE MANAGER
Beacon of Hope (BOH), a Division of Catholic Charities Community Services (CCCS), is seeking a Senior Case Manager to provide services to clients living in its Bronx Apartment Programs. The Senior Case Manager assists the Program Director with oversight of the office and supports all program staff. The Senior Case Manager provides support services to clients living with Serious Mental Illness. The Senior Case Manager will serve as a liaison for all other services required by clients, perform environmental assessments, and escorts clients to appointments. The Senior Case Manager performs work in accordance with Office of Mental Health regulatory requirements and BOH Policies and Procedures.
Works Monday through Friday, 9:00 AM - 5:00 PM. Apartment Program work requires home visits in the community.
Salary is $30.29 an hour to $31.25 an hour ($63,000 - $65,000 annually. Pro-rated on time actually worked during the year.) Salary is commensurate with educational attainment and experience. The position is located at 2510 Westchester Avenue, Bronx, NY 10461.
Job Requirements:
โ€ข Bachelor's Degree with two years Case Management experience, or a High School Diploma or its equivalent with five years of Case Management experience
โ€ข Two years of Case Management experience working with mentally Ill and/or substance use population
โ€ข Ability to work cooperatively and patiently with others.
โ€ข Ability to work well independently and as part of a team.
โ€ข Excellent organizational, time-management and communication skills.
โ€ข New York State Driver's License preferred
