Job Description
Catastrophic Case Manager
Your goal is to provide for quality, cost-effective outcomes for the client through collaboration and communication with the client, the family, the physician, and other members of the healthcare team.
Responsibilities:
โ€ข Process the case utilizing care management model and tools
โ€ข Develop a plan of care plan through interdisciplinary collaboration, which identifies realistic short-term and long-term goals
โ€ข Identify, secure, and coordinate the services and resources to implement the plan
โ€ข Monitor services being provided for continued appropriateness
โ€ข Evaluate the clients progress, the plan, and the appropriateness of the services provided, with modifications as necessary
โ€ข Promote the client's/family's self-advocacy skills so achieve maximum self-sufficiency
Requirements:
โ€ข Active CA Registered Nurse License
โ€ข Bachelors of Science degree in Nursing preferred
โ€ข CCM, CRRN, CDMS, or CRC preferred
โ€ข 2 years minimum related experience in catastrophic case management with worker's compensation
โ€ข Basic knowledge of computer and Microsoft Office products
โ€ข Valid driver's license and liability insurance
โ€ข Demonstrate effective, accurate, and timely verbal and written communication in all work situations
โ€ข Ability to organize and evaluate workload and prioritize projects to ensure deadlines are met
โ€ข Detail oriented with the ability to complete multiple, simultaneous projects with accuracy and efficacy
