Hybrid RN Case Manager
Hybrid RN Case Manager role focused on coordinating care for aging adults living independently at home. Community-based nursing opportunity centered around care planning, continuity of care, and service coordination. No bedside nursing responsibilities required. Active Massachusetts RN license REQUIRED. Minimum 2 years RN experience REQUIRED. Home care experience preferred. Strong communication and organizational skills REQUIRED.
