Hybrid RN Case Manager

Hybrid RN Case Manager role focused on coordinating care for aging adults living independently at home. Community-based nursing opportunity centered around care planning, continuity of care, and service coordination. No bedside nursing responsibilities required. Hybrid structure combining community patient visits with remote follow-up and care coordination work. Mission-driven senior care environment with strong interdisciplinary collaboration.

Two remote workdays weekly. No bedside nursing demands. No nights, weekends, or call responsibilities. Strong autonomy and independent workflow. Meaningful long-term impact with senior population. Stable weekday schedule supporting work-life balance. Relationship-focused nursing environment.

Monday-Friday schedule. 8:00 AM-4:00 PM hours. Three community-based days weekly. Two remote workdays weekly. No nights. No weekends. No call responsibilities.

Base salary: $84,000-$108,000 annually based on experience. Health insurance with 75% employer-paid premium. Dental and vision insurance. 26 combined PTO days. 13 paid holidays.
