POSITION OVERVIEW
โ€ข Hybrid RN Case Manager role supporting aging adults through community-based care coordination and individualized care planning
โ€ข Opportunity to work independently without bedside nursing demands
โ€ข Role focused on continuity of care, service coordination, and long-term patient support within home-based setting
โ€ข Hybrid structure combining community visits with remote care coordination responsibilities

SCHEDULE

โ€ข Base salary: $84,000-$108,000 annually based on experience

Requirements
โ€ข Active Massachusetts RN license REQUIRED
โ€ข Minimum 2 years RN experience REQUIRED
