Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Registered Nurse (RN) Case Manager in Beverly, Massachusetts.
POSITION OVERVIEW
Hybrid RN Case Manager role supporting aging adults through community-based care coordination and individualized care planning
Opportunity to work independently without bedside nursing demands
Role focused on continuity of care, service coordination, and long-term patient support within home-based setting
Hybrid structure combining community visits with remote care coordination responsibilities
Mission-driven senior care environment with strong interdisciplinary collaboration

Hybrid schedule with two remote workdays weekly
No bedside nursing responsibilities
No nights, weekends, or call

SCHEDULE
Monday-Friday schedule
8:00 AM-4:00 PM hours
Three community-based days weekly
Two remote workdays weekly
No nights
No weekends
No call responsibilities
COMPENSATION & BENEFITS
Base salary: $84,000-$108,000 annually based on experience

ROLE RESPONSIBILITIES
Develop individualized patient care plans
Coordinate community-based services and continuity of care
Conduct patient visits within community and home settings
Manage follow-up and ongoing care coordination remotely
Collaborate with interdisciplinary care teams
Support aging adults living independently at home

Requirements

Active Massachusetts RN license REQUIRED
Minimum 2 years RN experience REQUIRED
Home care experience preferred

Comfortable working independently in community settings REQUIRED

Comfortable with hybrid remote/community workflow REQUIRED
