POSITION OVERVIEW
• Hybrid RN Case Manager role supporting aging adults through community-based care coordination and individualized care planning
• Opportunity to work independently without bedside nursing demands
• Role focused on continuity of care, service coordination, and long-term patient support within home-based setting
• Hybrid structure combining community visits with remote care coordination responsibilities
