Position Summary

Tampa Family Health Centers is seeking a Family Practice Advanced Practice Registered Nurse to join our primary care team.

The APRN will provide comprehensive care to patients across the lifespan, including preventive care, acute treatment, chronic disease management, patient education, and care coordination. This position works collaboratively with physicians, clinical support staff, and other members of the care team within TFHC’s Patient-Centered Medical Home model.


Essential Responsibilities
  • Perform patient histories, physical examinations, and clinical assessments
  • Diagnose and manage common acute and chronic health conditions
  • Order, review, and interpret laboratory, imaging, and diagnostic testing
  • Develop treatment plans, prescribe medications, and coordinate follow-up care
  • Provide preventive care, health screenings, and patient education
  • Coordinate referrals and collaborate with interdisciplinary care team members
  • Maintain complete and accurate electronic health record documentation
  • Participate in daily care team huddles and quality-improvement initiatives
  • Support efforts to close care gaps and improve patient outcomes
  • Mentor or serve as a preceptor to APRN, PA, and nursing students when appropriate
  • Protect patient privacy, dignity, confidentiality, and safety
  • Comply with HIPAA, HRSA, federal, state, accreditation, and TFHC requirements
  • Float between TFHC locations when operationally necessary

Qualifications
  • Graduate of an accredited Advanced Practice Registered Nurse program
  • Current and unencumbered Florida APRN license
  • Current Family Nurse Practitioner or appropriate primary care board certification
  • Current Basic Life Support certification
  • Prescriptive authority in accordance with Florida law
  • Minimum of one year of APRN experience preferred
  • Strong clinical judgment, communication, and teamwork skills
  • Ability to work effectively with patients from diverse social, cultural, and economic backgrounds
  • Proficiency with electronic health record systems
