Work Location/Program Overview:

The Ascentria Care Alliance Young Parent Program empowers teen mothers and their young children living in multicultural congregate settings by helping them develop the skills and independence needed to thrive in the community. Our dedicated staff work closely with community partners—including DCF, schools, courts, and therapists—to provide comprehensive support and collaboration. Through this vital work, staff have the opportunity to make a meaningful and lasting difference in the lives of young families.

Position Overview:
The Child and Family Services Case Worker provides clinical case management to children and families, focusing on achieving a permanent, long-term life plan for each child. This includes outreach, intensive case management, and direct services while collaborating with various systems and advocating for the child’s needs. The CFS Case Worker is also responsible for developing and completing assessments, service plans, treatment plans, progress notes, and court reports in a timely manner.

Here’s what we’re looking for:

  • A Bachelor's Degree in Social Work or a related field
  • 1-3 years of experience working with children and families
  • Bilingual in Spanish and English (preferred)
  • A valid driver’s license and the ability to transport minors if needed
  • Candidates who are selected for hire must successfully complete all required background checks after starting their employment.

  • Strong computer skills, including Microsoft Word and email

  • Experience working with children who have social/emotional/physical challenges (preferred)
  • Knowledge of federal, state, program, and agency regulations (preferred)
