Water’s Edge Rehab & Nursing Center at Port Jefferson is seeking a detail-oriented and personable Admissions Coordinator to join our team. This role is ideal for someone who thrives in a fast-paced healthcare environment and is passionate about providing a seamless admissions experience for residents and their families.


Position Overview

The Admissions Coordinator is responsible for managing the intake and admission process, serving as a key point of contact for referral sources, hospitals, families, and internal departments. This individual plays a vital role in ensuring efficient transitions into the facility.


Key Responsibilities
  • Coordinate all aspects of the admissions process from initial referral through resident admission
  • Communicate with hospitals, case managers, families, and referral sources
  • Review clinical and financial documentation to ensure eligibility and appropriateness for admission
  • Verify insurance coverage and obtain necessary authorizations
  • Collaborate with interdisciplinary teams to facilitate smooth and timely admissions
  • Maintain accurate records and census tracking
  • Provide facility tours to prospective residents and families
