As a Regional Admissions Liaison, you will play a crucial role in ensuring seamless admissions processes for our short-term and long-term care patients. Responsibilities:
  • Coordinate and facilitate the admissions process for patients transitioning from acute care settings to our skilled nursing facility
  • Collaborate with healthcare providers and families to ensure a smooth transition for patients

  • Conduct assessments and screenings to identify patient needs and develop personalized care plans
