Dependable Health Services is seeking a compassionate, organized, and experienced Admissions Registered Nurse (RN) to join our growing Home Health team in the Phoenix metropolitan area.
As an Admissions RN, you will play a critical role in helping patients begin their home health journey by conducting comprehensive Start of Care assessments, developing initial plans of care, and ensuring a smooth transition into services. Position Summary
As a Home Health Admissions RN, you will serve as the first clinical point of contact for patients entering home health services. You will perform Start of Care assessments, establish care plans, coordinate services, and educate patients and families while ensuring compliance with Medicare and agency standards.
Essential Responsibilities
Patient Assessments
  • Conduct comprehensive Start of Care (SOC) assessments
  • Evaluate patient needs, functional status, and home environment
  • Identify clinical concerns and establish appropriate interventions

Care Planning
  • Develop individualized plans of care based on physician orders and patient needs
  • Coordinate services and communicate care goals with the interdisciplinary team

Patient & Family Education
  • Educate patients and caregivers regarding diagnoses, medications, treatments, and home health services
  • Provide ongoing support and guidance to promote successful outcomes

Clinical Documentation
  • Complete accurate and timely OASIS documentation
  • Maintain compliance with agency, state, and federal regulations
  • Communicate patient status and care recommendations to physicians and care team members

Care Coordination
  • Collaborate with physicians, therapists, social workers, and other healthcare professionals
  • Facilitate smooth transitions into home health services

Territory Requirements
This position is responsible for admissions throughout the Central Phoenix and West Valley service areas, including:
  • North East Valley
  • North West Valley
  • South West Valley

Candidates must be comfortable traveling throughout the assigned territory and managing admissions across multiple locations within the Phoenix metropolitan area.
Qualifications
Required
  • Current Arizona Registered Nurse (RN) license in good standing
  • Recent home health experience
  • Proficiency with OASIS documentation
  • Current CPR certification
  • Valid Arizona driver's license, current auto insurance, and reliable transportation
  • Arizona Fingerprint Clearance Card
  • Strong assessment, communication, and organizational skills

Preferred
  • Bilingual English/Spanish
  • Experience completing Start of Care admissions
  • Knowledge of Medicare home health regulations

Education
  • Associate Degree in Nursing (ADN) required
  • Bachelor of Science in Nursing (BSN) preferred
