Responsibilities

As an Admissions Registered Nurse, you will:

  • Complete 1–3 Start of Care (SOC) visits each day throughout Monmouth, Middlesex, Somerset, and Union Counties
  • Conduct comprehensive patient assessments and accurately complete OASIS documentation
  • Develop individualized plans of care in collaboration with physicians and the interdisciplinary care team
  • Deliver skilled nursing care, including medication administration, wound care, treatments, and patient education
  • Educate patients and caregivers on disease management, medications, and home safety
  • Monitor patient conditions and communicate changes appropriately
  • Ensure documentation is accurate, timely, and compliant with regulatory standards

Qualifications

  • Current Registered Nurse (RN) license in the State of New Jersey
  • Minimum of one year of nursing experience (Home Health experience preferred)
  • Experience completing OASIS documentation and Start of Care visits strongly preferred
  • Comfortable using electronic medical records (EMR); Homecare Homebase (HCHB) experience is a plus
  • Excellent assessment, communication, and time management skills
  • Passion for providing exceptional, patient-centered home health care

Additional Information

Base Pay, depending upon qualifications, status, and subject to negotiation:

$65.00-$75.00 per point ($101,400- $117,000 annual earning potential)

$20,000 Sign-On Bonus for Full-Time RNs in Monmouth County Only!
