Home Health Admissions RN (PRN) โ€“ OASIS Required

Viva Life Home Health & Home Care is seeking an experienced Home Health Admissions Registered Nurse (RN) who excels at completing Start of Care (SOC) visits and OASIS assessments. This is not a typical field nursing positionโ€”we are looking for a clinician who is confident working independently, comfortable making sound clinical judgments, and understands the importance of accurate OASIS documentation.

As one of the first clinicians our patients meet, you will play a vital role in establishing the foundation for exceptional patient care while ensuring compliance with Medicare home health regulations and quality standards.

What You'll Do
  • Independently complete comprehensive Start of Care (SOC) visits and OASIS assessments.
  • Perform Resumption of Care (ROC), Recertification, Discharge, and routine skilled nursing visits as needed.
  • Conduct comprehensive nursing assessments and develop individualized plans of care.
  • Coordinate care with physicians and the interdisciplinary team.
  • Provide skilled nursing services, including wound care, medication management, catheter and ostomy care, injections, disease management, and patient education.
  • Recognize changes in patient condition and communicate appropriately with providers and clinical leadership.
  • Complete accurate, timely, and compliant documentation in the agency's EMR.
  • Maintain compliance with Medicare Conditions of Participation, agency policies, HIPAA, and infection prevention standards.
Qualifications Required
  • Current, unrestricted Texas RN license.
  • Minimum of one year of recent home health experience (two or more years preferred).
  • Demonstrated ability to independently complete Start of Care (SOC) OASIS assessments.
  • Strong knowledge of Medicare home health documentation requirements.
  • Current BLS/CPR certification.
  • Valid driver's license, reliable transportation, and proof of auto insurance.
  • Excellent assessment, critical thinking, and time management skills.
Preferred
  • Extensive OASIS experience.
  • Wound care experience.
  • Home health EMR experience.
  • Familiarity with PDGM and quality measures.
  • Outstanding communication and patient education skills.
