We are hiring an Admission Registered Nurse (RN) for a Home Health position providing skilled nursing care to patients in their homes in the Washington, PA area. This position is eligible for a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year!

What You’ll Do:

  • Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you

  • Conduct comprehensive start-of-care assessments, including clinical evaluations, medication reconciliation, and development of initial care plans aligned with physician orders

  • Educate and guide patients and families through the admission process, setting expectations for care, reviewing diagnoses and treatment plans, and ensuring they feel informed and supported from day one

  • Apply strong clinical judgment and critical thinking to coordinate admissions efficiently, address barriers to care, and ensure accurate, timely, and compliant EMR documentation

  • Initiate evaluations to other disciplines when indicated

Admission RN Qualifications

  • Active Registered Nurse, RN license in the state you work

  • One year of RN experience and the clinical expertise to deliver quality patient care

  • Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
