Heartland Home Health is looking for a Full Time Weekend Admissions RN to welcome patients into our home health program in Ft. Wayne, ensuring a smooth, professional, and compassionate admission experience from start to finish.
What You'll Do as a Home Health Admissions RN:
  • Conduct comprehensive initial assessments and evaluations to determine patient eligibility and care needs.
  • Initiate the Plan of Care in accordance with physician orders, clinical standards, and regulatory guidelines.
  • Document all patient assessments, clinical findings, and admission records accurately and promptly.
  • Serve as a liaison between referral sources, patients, families, and the interdisciplinary team to ensure a smooth and supportive onboarding process.
  • Maintain compliance with all applicable federal, state, and agency policies.
  • Uphold professional nursing standards while delivering compassionate, hands-on patient care.

  • RN Licensure - Indiana
  • Schedule: Friday, Saturday, Sunday (12 Hour Shifts); Monday (4 hour shift, remote except 1/month)

About You
Qualifications - What You'll Bring:
  • Active RN license in the state of employment (or eligibility to obtain).
  • 1+ year of high acuity nursing experience
  • A compassionate, flexible, and patient-centered approach to care.
  • Comfort providing care in a variety of home and community settings.
  • Strong organizational, communication, and clinical decision-making skills.
  • Valid driver's license, automobile insurance, and reliable transportation.
  • Current CPR certification.

Preferred Experience (Not Required):
  • 1 + year experience in home health, hospice, ICU, geriatrics, Med-Surg, ER, ICU, PACU, telemetry, cardiac or a related clinical setting.
  • Admissions RN, wound care, float pool, or travel nursing
