Job Summary for the Admissions Director position:

We are seeking an experienced Admissions Director to join our team! As the Admissions Director, you will be responsible for coordinating communications between hospital discharge planners, case managers, social workers, medical staff, and caregivers regarding discharge from the hospital and admission to a skilled nursing facility. You will assess referred patients to determine appropriate placement based on the facility's admission criteria and create and foster positive relationships with all community referral sources and potential admissions into Sterling.

Responsibilities for the Admissions Director position:

  • Manage leads in primary referring hospitals, ensuring appropriate, timely responses on behalf of the facility

  • Review documentation of potential new admissions, conducting in-person assessments as needed to determine appropriateness

  • Complete insurance verification

  • Ensure a smooth discharge process for patients transitioning from a hospital or other setting to a skilled nursing facility

  • Develop strong relationships with key referral hospital personnel and identify new opportunities for partnering and growth

  • Manage referral source relationships and support the implementation of the organization's marketing plan

  • Conduct regular visits to referring hospitals, building and maintaining relationships with discharge planners, case managers, physicians, and other referral sources

  • Maintain thorough knowledge of the market in general, competitors, and acuity trends

  • Assist in meeting the referral and admissions goals for assigned hospitals

  • Expand the number of referral sources in the market

  • Maintain current referral sources through relationship development

  • Keep accurate records and create reports of activities and outcomes weekly

  • Establish strong and successful relationships with referral sources throughout the area

Requirements for the Admissions Director position:

  • 2 years of experience in the Admissions department in a long-term care setting preferred

  • Must possess excellent organizational and communication skills
