LPN-Community Care Coordinator Job Description

The LPN-Community Care Coordinator (CCC) is in a leadership role in managing home care services provided at an independent living community in Raleigh and is responsible for the overall care provided by Aware in the community.

Main Responsibilities

• The Community Care Coordinator (CCC) has overall responsible for the Community Building. Must know all the Community Managers and establish good rapport with them. Check in with them regularly to ensure they are happy and feel we go out of our way to communicate with them.
• Perform resident care assessments and care plans to be approved by the assigned RN to the community.
• Provide leadership to the caregivers (aides) providing services to the residents of the community including initial orientation and teaching documentation requirements (note: we use a cloud-based software system called CINCH to electronically document).
• Provide nursing services as to residents as directed by their care plan.
• Ongoing oversight of all aides to assure each of them has a full understanding of community protocols and documentation responsibilities.
• Recommend and implement changes to client and aide schedules with input from team members, as appropriate.
• Responsible for exceptional care for all Community residents receiving services and assure resident/family satisfaction remains high.
• Assure aide satisfaction remains high.
• Address all issues promptly and document according to ASC policies.

Reports/Communications

• Attend weekly company team meetings.
• Weekly summary reports on the community – managers, aides, and client satisfaction. Any issues and if so, recommended action plans to address.
• Communicate all changes in resident condition to assigned community RN and Manager of Care Coordination.

Job Requirements

• Must hold an unencumbered license in NC as a Licensed Practical Nurse (LPN).
• Ability to independently organize work and provide effective home care services.
• Ability to ensure appropriate resident (client) care as supervised by the homecare Registered Nurse, and document thoroughly, accurately, and completely on a timely basis.
• Effective communication skills (oral and written).
• Adept at using technology for documentation and caregiver/client scheduling (note: we do not use paper).
• At least two (2) years of geriatric home health or homecare experience preferred.
• Ability to stand, walk, concentrate, climb stairs, bend, pull, reach, lift at least 25 pounds, squat, twist, and push without restrictions.
• No current physical or mental restrictions ordered by a medical professional that limit any of the above qualifications.
• Must live within 15 miles of zip code 27613.

Compensation (commensurate with experience):

• Starting at $23/hr.*
• * If benefits are declined, the hourly wage will be increased in the amount equivalent to the annual cost of benefits ($4/hr.).
