Job Title: Patient Advocate Housing Coordinator

Department: Community Health

Reports to: Social Services Supervisor

Effective: 3.8.2022

SUMMARY: We are a community health center looking for a Housing Case Manager to provide support services to our patients experiencing housing insecurity. The Housing Case Manager will be responsible for assessing patients’ housing needs, connecting them to appropriate housing resources, and providing ongoing case management to ensure successful housing stabilization.DUTIES AND RESPONSIBILITIES:
  • Conduct assessments to identify patients experiencing housing insecurity and determine appropriate interventions
  • Connect patients and community members to appropriate housing resources, such as emergency shelters, transitional housing, and permanent supportive housing
  • Coordinate with community partners, including housing providers and social service agencies, to identify available resources and support services
  • Provide ongoing case management to ensure successful housing stabilization, including regular check-ins, goal-setting, and problem-solving
  • Work collaboratively with the healthcare team to ensure patients receive comprehensive, integrated care
  • Maintain accurate and timely documentation of patient interactions, progress, and outcomes in electronic health records
  • Perform Housing Assessments in Hennepin and Ramsey counties to place patients and staff on permanent supportive housing waiting lists
  • Participate in quality improvement activities to enhance housing services and outcomes for patients
  • Attend meetings and trainings to stay current on housing policies, regulations, and best practices
  • Responsible for care coordination tasks as directed by the Community Health Manager; including finding resources and working with patients to identify and resolve the barriers that prevent them from completing medical, dental, and behavioral health treatment plans
  • Responsible for acting as a social resource expert at NACC assisting providers and other staff with patient’s social needs
  • Acts as part of the outreach team promoting NACC in the community; attend community health fairs, events and expos, to share information, promote NACC services and improve community health
  • Works to enroll uninsured patients for medical insurance and maintains up-to-date community resources
  • Helps to maintain current community resource information in an organized and accessible format, including Insurance, pharmacy, traditional healing, counseling, housing, tribal registration, transportation, food assistance
  • Meets with patients in the clinic to assess and assist with the patient's social needs— performs both in reach and outreach to connect patients to social services
  • Acts as an advocate for patients and works to promote community health needs
  • Performs other related duties as assigned by management.
SUPERVISORY RESPONSIBILITIES:

This job has no supervisory responsibilities.

QUALIFICATIONS:
  • Minimum of two years of experience in housing case management or related field
  • Knowledge of housing policies, programs, and resources, as well as housing-related health issues
  • Ability to work independently and as part of a team, with excellent interpersonal, communication, and problem-solving skills
  • Proficiency in electronic health records and other relevant software
  • Strong commitment to social justice, health equity, and serving underserved populations—experience working with indigenous people or communities preferred.
  • Bilingual skills in English and Spanish preferred
  • Associate's degree (A. A.) or equivalent from a two-year College or technical school; or six months to one year related experience and/or training; or an equivalent combination of education and experience
  • Computer skills required: Internet Software; Spreadsheet Software (Excel); Word Processing Software (Word); Electronic Mail Software (Outlook);

Occasional exposure to bloodborne and airborne pathogens or infectious materials

While performing the duties of this job, the noise level in the work environment is usually moderate

The employee must: occasionally lift and /or move more than 10 pounds; frequently lift and/or move up to 10 pounds
