Job Summary: The Assisted Outpatient Treatment (AOT) Monitor is mainly responsible for monitoring treatment compliance for individuals on an AOT order. The Monitor establishes strong working relationships with the OCHN provider network and hospital system for education, coordination between the Probate Court and the mental health system, and support to ensure compliance of the AOT court order. The Monitor is also responsible for attending the Deferral Conferences and assisting the person with initial access to services.

Essential Functions:

  • Maintains a caseload for the purpose of monitoring treatment compliance of individuals on an AOT order.
  • Works closely with OCHN’s provider network.
  • Updates the Probate court on treatment compliance through written or oral report.
  • Attends Probate Court hearings and provides the court with information on AOT compliance.
  • Files non-compliance petitions as needed.
  • Assists OCHN’s provider network regarding renewing an order.
  • Maintains accurate and timely documentation in the Electronic Health Record.
  • Interviews potential referrals, participates in individual’s discharge planning from the hospital.
  • Attends Deferral Conferences, explores services needs and coordinates accordingly.
  • Tracks and maintains data on all court orders and other data points, as identified.
  • Reviews Electronic Medical Record for current hospitalization, anticipated discharge date, insurance status, treatment engagement, etc.
  • Develops and maintains working relationships with community partners, including the Oakland County Probate Court, crisis providers, hospitals, provider network, law enforcement, etc.
  • Provides training and education for Probate Court, provider network, community members and law enforcement on AOT.
  • Collaborates with other OCHN teams regarding department cross-over for assistance.
  • Participates in agency meetings, including AOT Workgroup / Steering Committee.

Job Requirements and Qualifications:

Education:

  • Bachelor's degree in Social Work, Psychology or Criminal Justice.

Training Requirements (licenses, programs, or certificates):

  • LLBSW / LBSW, preferred.

Experience Requirements:

  • At least two years in the behavioral health field working, focused on adults with serious mental illness.

Job Specific Competencies/Skills

  • Ability to establish and maintain rapport.
  • Demonstrated ability to work effectively in a team environment, as well as independently.
  • Ability to utilize Motivational Interviewing, as well as trauma-informed practices.
  • Have knowledge of health insurance practices and be able to direct clients in using services available to them through their health plans.

Knowledge Requirements:

  • Knowledge of the justice system, especially Probate Court proceedings.
  • Knowledge of services and support in a crisis continuum of care.
  • Knowledge of the Petition and Certification processes.
  • Knowledge of Recipient Rights.
  • Knowledge of LOCUS, preferred.
  • Knowledge of public mental health service delivery systems in Oakland County.
  • Knowledge of HIPAA.

Additional Information:

(Travel required, physical requirements, on-call schedules, etc.):

  • Must have available means of transportation to and from OCHN and for required offsite meetings or site visits.
  • Must be available for meetings and events which may occur outside of standard office hours.
  • Must travel to hospitals, court, and other community locations.
  • Hybrid (onsite/remote) work schedule available.
