Mid-Level Provider

Department: Clinical Job Status: Full Time

FLSA Status: Exempt Reports To: Director of Operations COO

Grade/Level: Master’s and/or Doctorate Degree Amount of Travel Required: < 30%

Work Schedule: Positions Supervised: None

8 a.m. to 5:30 p.m., Monday - Thursday. 8 a.m. to 12:00 p.m. on Friday.

7 a.m. to 4:30 p.m., Monday-Thursday (Eudora and Portland Only)

However, additional hours may be required to complete duties, including Friday afternoon and Saturday clinic.

POSITION SUMMARY:

Mid-Level Providers work as part of the care team as licensed independent practitioners who are responsible for managing health problems and coordinating health care for patients with a specific focus on health promotion, disease prevention, and quality outcomes. Mid-level providers perform a variety of direct patient services including assessment, diagnosis, implementation of treatment, evaluation, and monitoring the general practice management plan for clinic patients. Mid-Level Providers perform these functions in collaboration with a licensed physician in a primary health care setting. Mid-Level Providers are involved in continuous quality improvement practices and measures as set by Mainline Health Systems, Inc. and in which the practice focus is based on a Patient Centered Medical Home model of care.

SPECIFIC JOB DUTIES AND RESPONSIBILITIES:

  1. Provides primary medical care by caring for all patients in accordance with the mid-level providers state board approved certification or specialty with the goal of providing primary health care services including preventative health measures.

  1. Develops a plan of care for each patient, including complete medical history, physical examination, diagnosis, appropriate treatment and/or referral, including hospitalization where necessary, order and interpret lab and diagnostic imaging as deemed necessary.

  1. Uses all available resources in diagnosis and treatment, such as laboratory and radiological testing, in an appropriate and cost efficient manner.

  1. Collaborates with consulting physicians, nurses, patients, and patient’s families concerning treatment and care of patients.

  1. Refer MHSI patients which require specialty services. The mid-level provider maintains responsible for the patients care by assuring that continuity of care is provided.

  1. Attends all medical staff meetings, monthly clinic meetings, quarterly provider meetings, and participates in the community health center’s QI program.

  1. Performs other necessary duties as required by MHS to meet the goals of providing quality, primary healthcare.

  1. Communicates concerns or necessary information to the care team. Mid-level providers are required to participate in daily clinic team huddles.

  1. Prescribes appropriate treatment, pharmacological and nonpharmacological modalities. Mid-level provider is responsible for informing and educating patients and appropriate family or caregiver to allow patient autonomy. All mid-level providers with prescriptive authority may prescribe medications with their scope of practice and standards of care in accordance with their collaborative practice agreement, and MHSI policy.

  1. Patient, family, and community education and counseling.

  1. Consults with other healthcare team members as necessary.

  1. Formulates, initiates, and monitors patient management plans using clinical protocols and best practice.

  1. Renders emergency care and treatment, including but not limited to, advanced cardiac life support, administering IV fluids, administering medications, and treatment of injuries and wounds and any other necessary procedure within the mid-levels scope of practice that privileges have been granted by MHSI Board of Directors.

  1. Maintains all patient records to comply with required standards, reviews records regularly, and obtains physician counter signatures where necessary. Progress notes are expected be completed within 3 business days, and are required to be completed within 30 days.

  1. Serve as part of the care team; following and implementing workflows that support and sustain MHSI Patient Centered Medical Home model of care.

  1. Willingness to attend and/or complete continuing education related to this position and/or any other topics as deemed necessary to MHSI.

  1. Completion of continual job training processes set forth by Mainline Health Systems, Inc.

  1. Maintains safety-infection measures and supervises the safety within the local clinical settings.

  1. Observe all safety/infection control policies and procedures and attends safety/infection control committee training as required.

REPORTING RELATIONSHIPS:

The Mid-Level Provider reports to, is supervised by, and is evaluated by the Director of Operations and/or COO.

EVALUATION:

The evaluation of work performance will be on-going and will be carried out by the COO. It will include the specific duties and responsibilities of this position description plus employee attitude and general working behavior. Formal evaluation will normally be performed annually, but may be initiated at other times by the COO, Chief Clinical Officer, and/or CEO.

QUALIFICATIONS:

  1. Graduation from an accredited school for Physician Assistant or Nurse Practitioner Program recognized by the State of Arkansas.
  2. Must have an Arkansas license and maintain current certification to practice in Arkansas.
  3. The ability to relate with effectiveness to patients and providers.
  4. Must be certified in BLS and ACLS.
  5. Sufficient experience to carry out the duties of this position.
  6. Prescriptive authority from state board with active DEA registration.
  7. Computer skills with capabilities to utilize Excel spreadsheets, graphs, and Word documents preferred.
  8. Ability to learn and utilize electronic record keeping systems such as electronic medical record required.
  9. The ability to work in a constant state of alertness and in a safe manner.

Continuing Professional Education must be obtained and documented annually in accordance with MHSI’s Continuing Professional Education and Arkansas licensing requirements.

Appointment to the position is for the term of the grant supporting the position, as long as job performance meets the standards developed by the Board of Directors. Raises in pay may be made available upon recommendation of the Supervisor, CEO and approval of the Board.

The Mid-Level provider must be able and willing to travel to any MHSI clinic location upon request to provide services and ensure MHSI patient’s access to care.
