Summary: The Medical Assistant supports the Care Management Team medical providers and ensures that all patients receive quality care while maintaining compliance with all federal grants, HIPAA, and Hunter Health Clinic policies and procedures.

 

Job Responsibilities
Care Management Responsibilities:
 Assists in implementing care management programs.
 Coordinates patient discharge and post-discharge actions based on program requirements.
 Partners with providers and care teams to complete designated office visits.
 Assists providers and care teams with improving value-based care data collection to satisfy program requirements.
 Documents attribution-based patient data into EHR and value-based care platforms, as directed.
 Prepares reports by collecting, analyzing, and summarizing results data and trends, compiling statistics.
 As requested, will assist in coordinating services such as arranging resources, including transportation and escort.
 Maintains confidentiality of patient and proprietary information.
 Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.

 

Medical Assistant Responsibilities:
 Assists Physicians, Mid-Levels, RN’s, and LPN’s with patient care, as requested.
 Utilizes data tracking boards to identify setbacks and opportunities for improvement at the end of each session.
 Documents Chief Complaint in EHR.
 Collects and documents (EHR) the Patient History prior to the patient being seen by medical provider.

 Prepares for patient visits to include identifying needs, setbacks, orders; room and exam preparation; rooming of the patient.
 Performs septic handwashing before and after rooming patients in exam room and procedures.
 Proper dismissal of patient to include education.
 Assists with telephone encounters documented daily in EHR.
 Documents patient encounters, phone calls, drug administration, and chart review in EHR.
 Communicates and collaborates professionally and effectively with medical providers, medical support staff, front desk staff, patient service representatives, patient care coordinators, and administration.
 Responsible for by-laws strict confidentiality of Medical Records.
 Attends designated meetings as an employee and medical staff member.
 Completes all assigned monthly trainings.
 Other duties as assigned.

 

Qualifications:
 1 to three years’ experience in a Medical Clinic setting, preferred.
 Certified or Registered Medical Technician, Medical Assistant and/or Certified Nurse Assistant, preferred.
 Prior experience with Electronic Medical Records software, preferred.
 Bilingual Spanish or equivalent, preferred.

 Finger Dexterity: Using primarily just the fingers to make small movements such as typing, picking up objects and pinching fingers together

 Average Visual Abilities: Specific vision abilities required by this job include close vision, color vision, and ability to adjust focus.
 Working Conditions: Works in a well-lit, climate-controlled environment. The noise level in the work environment is usually high. Potential exposure to infectious disease is possible.
 Physical Strength: The employee must occasionally lift and/or move up to 35 pounds.
 Mathematical Ability: Ability to add and subtract two-digit numbers and to multiply and divide with 10's and 100's.
