Centra Medical Center is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) for a full-time position within our remote hospice care program. • Conduct comprehensive psychosocial assessments to identify clients' mental health needs, strengths, and resources, with a focus on their unique experiences during the hospice process.
• Deliver individual, family, and group therapy using a variety of evidence-based treatment modalities tailored to the specific challenges faced by hospice patients.
• Develop, implement, and regularly update individualized treatment plans to address the dynamic needs of clients, ensuring alignment with their goals and values.
• Facilitate group therapy sessions, psychoeducation classes, and support groups focused on grief, loss, and coping strategies.
• Coordinate closely with a multidisciplinary team, including physicians, nurses, and other specialists, to ensure holistic and coordinated patient care.
• Provide crisis intervention and risk assessments, employing safety planning as necessary to address urgent mental health situations.
• Maintain accurate and timely documentation of client interactions and treatment plans in compliance with legal and ethical standards.

In this role, you will work primarily with individuals and families experiencing the emotional turmoil associated with end-of-life care. Our client focus ranges from elderly patients managing chronic illnesses to younger individuals providing care for loved ones. You will play an instrumental role in helping them navigate grief, anxiety, and other mental health challenges, ensuring they receive the comfort and support they need.

• A Master’s degree in Social Work from an accredited program.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of direct clinical experience in a mental health or outpatient setting, preferably in hospice or palliative care.
• Strong understanding of evidence-based therapeutic techniques, particularly in trauma-informed care approaches.
• Demonstrated excellent interpersonal and communication skills, with a strong ability to connect emotionally with clients facing difficult circumstances.

• Familiarity with hospice and palliative care systems, including an understanding of the stages of grief and the psychosocial aspects of terminal illness.
• Experience in working collaboratively within a multidisciplinary team setting.
• Knowledge of local resources and support services available for hospice patients and their families.

The salary for this position ranges from USD 80,000 to 100,000 annually, commensurate with experience and qualifications. Additionally, a sign-on bonus is available to qualified candidates.
