• Conduct thorough psychosocial assessments to identify the emotional, social, and financial needs of patients and their families in hospice care.
• Provide individualized counseling interventions, including supportive therapy and crisis intervention tailored to each family's unique circumstances.
• Develop detailed treatment plans that outline specific goals and strategies for addressing patients' needs, including emotional support and coping strategies for family members.
• Collaborate closely with healthcare professionals, including nurses and physicians, to ensure a multidisciplinary approach to patient care that is client-centered and holistic.
• Facilitate virtual grief counseling and support groups to assist families dealing with loss and bereavement, while using evidence-based modalities to promote healing.
• Document and maintain accurate records of patient interactions, treatment plans, and progress, adhering to ethical and legal standards expected in clinical practice.
• Utilize evidence-based practices and therapeutic modalities such as cognitive-behavioral therapy (CBT) and mindfulness techniques to support patient and family resilience.
• Engage in ongoing professional development to stay current with best practices in hospice and palliative care.

This role primarily supports individuals with terminal illnesses and their families, targeting a range of emotional challenges that accompany end-of-life care. You will address mental health concerns that can arise during this sensitive time—such as anxiety, depression, and emotional distress—helping to foster an environment of healing and comfort.

• A Master's degree in Social Work from an accredited institution.
• Current state licensure as a Licensed Independent Clinical Social Worker (LICSW) is required.
• A minimum of two years of clinical experience in a mental health or outpatient care setting, preferably in hospice or palliative care.
• Strong comprehension of evidence-based therapeutic modalities and a commitment to continuous learning and application of innovative techniques in clinical practice.
• Exceptional communication, interpersonal, and assessment skills, with a deep empathy for patients and their families.

• Experience providing support in a hospice or end-of-life care setting.
• Familiarity with telehealth platforms and electronic health record (EHR) systems to streamline patient management and documentation.
• Cultural competency and the ability to work with diverse populations are highly valued in our approach to care.

We offer a competitive salary range of USD 80,000 to 100,000 annually, along with a sign-on bonus for qualified candidates.
