Job Description Summary:

The Physical Therapist is responsible for providing exceptional care to patients in their homes and within home health and hospice settings in Texas and New Mexico while reporting directly to the Director of Nurses in Texas and New Mexico.

Job Duties (include, but not limited to):

  • Perform patient assessments, develop treatment plans, and implement intervention techniques tailored to the needs of patients in both Texas and New Mexico.
  • Adhere to state regulations in Texas and New Mexico, agency policies, and professional standards of practice, maintaining excellence in patient care delivery.
  • Collaborate with interdisciplinary team members, including physicians, nurses, occupational therapists, and social workers, to facilitate seamless care coordination and enhance patient outcomes.
  • Seek guidance from the Physical Therapist Supervisor on challenging cases, ethical considerations, and professional development matters.
  • Maintain clear communication with management and external stakeholders to support organizational objectives.
  • Stay informed of current trends, research findings, and best practices in physical therapy and home health/hospice care, integrating evidence-based interventions into clinical practice as deemed appropriate.
  • Adhere to HIPAA Policies, ensuring compliance with state-specific privacy laws in both Texas and New Mexico to safeguard patient confidentiality.
  • Fulfill additional responsibilities as assigned, recognizing the distinct requirements of providing care in the home health and hospice settings across Texas and New Mexico.

Working Environment:

  • Prudent use of office supplies and equipment.
  • Ability to communicate effectively.
  • Possess manual dexterity and normal range of motion of all extremities.
  • May require occasional weekend, holiday, or overtime work.
  • Skills in problem-solving, considering patients' backgrounds, finances, and resources, are essential for delivering optimal care.
  • Reliable transportation for travel to patient homes and agency locations.
  • Compliance with dress code and professional demeanor.
  • Ability to communicate effectively in English.
Requirements:

Position Qualifications:

  • Licensure as a Physical Therapist in Texas or New Mexico, meeting one of the following requirements:
  1. Graduation from a physical therapist education program approved by the Commission on Accreditation in Physical Therapy Education (CAPTE) or successor organizations.
  2. Graduation from an education program outside the United States determined to be substantially equivalent to physical therapist entry-level education in the United States by an approved credentials evaluation organization.
  3. Meeting alternative licensure pathways as outlined by state regulations.
  • Minimum of five years of clinical experience as a licensed Physical Therapist, with demonstrated proficiency in home health and hospice settings preferred.
  • Prior experience in a supervisory or leadership role, with strong leadership, communication, and team-building skills.
  • Excellent clinical assessment, critical thinking, and problem-solving abilities.
  • Strong understanding of regulatory requirements, reimbursement guidelines, and documentation standards related to home health and hospice care.
  • Commitment to promoting a culture of excellence, integrity, and continuous improvement within the therapy team.
  • Ability to effectively manage multiple priorities, delegate tasks, and make sound decisions under pressure.
  • Proficiency in computer skills, including electronic medical records and productivity tracking software.
  • Current CPR certification and adherence to professional standards and practice.
  • Proficiency in electronic health records (EHR) and other relevant software applications.
  • Possess organizational skills, with an understanding of state-specific regulations governing home health and hospice services in Texas and New Mexico.
