Physical Therapist (Permanent Placement)

Location: Piscataway, NJ

Type: Full-Time | Permanent

Pay: $40.00/hr – $50.00/hr

Shift & Schedule: Monday – Friday | 8:00 AM – 4:30 PM

Position Overview

We are seeking a compassionate and dedicated Physical Therapist to join a rehabilitation team in a skilled nursing and post-acute care setting. This role is responsible for evaluating patients, developing individualized treatment plans, and delivering evidence-based therapy interventions designed to improve mobility, strength, balance, and functional independence.

Responsibilities

• Evaluate residents and develop individualized treatment plans.

• Implement evidence-based physical therapy interventions.

• Improve patient mobility, strength, balance, endurance, and functional independence.

• Monitor patient progress and adjust treatment plans as needed.

• Collaborate with physicians, nurses, occupational therapists, speech therapists, and interdisciplinary team members.

• Maintain accurate and timely clinical documentation.

• Educate patients, families, and caregivers on treatment plans and home exercise programs.

• Ensure compliance with facility, state, federal, and professional practice standards.

• Participate in care planning meetings and interdisciplinary team discussions.

• Promote patient safety and quality outcomes throughout the rehabilitation process.

Required Qualifications

• Graduate of an accredited Physical Therapy program.

• Current New Jersey Physical Therapist License or eligibility for licensure.

• Strong clinical evaluation and treatment skills.

• Excellent communication and patient care abilities.

Preferred Qualifications

• Experience in Skilled Nursing Facilities (SNF).

• Experience in rehabilitation or post-acute care settings.

• Experience working with geriatric and medically complex patients.
