POSITION SUMMARY:
Case Manager set clear, measurable goals to work towards housing permanency. Case Manager empowers clients to achieve their goals by creating a collaborative and supportive working relationship with clients based on empathy, accountability, and trust. Case Manager provides information, referrals, and coordinate services such as health care, job training, housing and immigration services, among others. Case Manager is responsible for developing strategies for engaging clients and creating awareness about services in the community and developing a path to re-housing.
POSITION DUTIES:
  • Case managers carry a caseload of 25 clients.
  • Ensure that everyone assigned is personally interviewed, assessed, within 24 hours of admission.
  • Conduct intakes and make assessments of residents' needs and coordinate referrals as needed.
  • Along with residents, develop individualized service plans with short- and long-term goals and review the plans bi-weekly
  • Monitor compliance with goals through regular weekly scheduled appointments. ILP is reviewed with the residents on a bi-weekly basis.
  • Maintain files in orderly fashion and up to date. Missing appts must be documented and remedy action to make up the appt as well.
  • Work closely with shelter staff to enforce client's rules and regulations
  • Hold conferences with the clients when rules are broken or disregarded.
  • Serve as an advocate with agencies and service providers to minimize barriers to clients receiving needed services.
  • Follow up with referral sources to ensure that clients receive, and access necessary services in a timely manner. Accompany a client to an appt when necessary.
  • Conduct apartment inspections to expedite re-housing.
  • Coach clients in job prep, housing availability, social services and other areas as needed.
  • Represent CP Unlimited at city and statewide coalitions, task forces and committees as assigned by supervisor
  • Participate in trainings and meetings, case conferences and training seminars as indicated by Administration
  • Comply with trainings as required by Department of Homelessness or Constructive Partnership Unlimited
  • Perform other duties as assigned by the Case Work Supervisor

REQUIREMENTS:
  • Bachelor's degree in human services (No experience required) or bachelor's degree in field outside of Human Services with one
    year experience in a Human service's program or congregate setting.
  • Demonstrate sensitivity and experience with and/or knowledge with disenfranchised communities including individuals who identify as lesbian, gay, bisexual, transgender, queer, and/or HIV-affected, and individuals with mental health issues and homelessness.
  • Experience with homeless population preferred
  • Must have good written and oral communication skills
  • Able to work under pressure, able to respond to a crisis, health or safety issue and take appropriate and immediate action
  • Must be able to work flexible hours including evening and weekends
  • Must be computer-literate and must have knowledge of Microsoft Office applications
  • CPR/First Aid Required
  • Narcan Certification required
  • Bilingual - Spanish, French and French creole preferred
