Average Hourly Rate: $25.00

(Includes wage, incentives, bonuses, overtime, shift differential, etc.)


$5,000 Sign-On Bonus

For applicants who have successfully completed the full 13 weeks of CVS Conservatorship or SSCC Permanency training as a CVS Specialist and are fully case-assignable at the time of hire.

Position Overview: Permanency Case Manager

In this impactful role, you will work directly with children in the Managing Conservatorship of DFPS and their families. You'll assess needs, develop plans, and coordinate services to help families safely reunify-or ensure every child achieves permanency in the most nurturing setting possible.


Key Responsibilities

As a Permanency Case Manager, you will:

  • Manage a caseload and guide families throughout the permanency process
  • Assess child and family needs, safety concerns, and strengths
  • Facilitate permanency planning and ensure safe, stable placements
  • Conduct ongoing searches for potential Kinship placements
  • Visit children and families monthly-or more often as needed
  • Provide post-reunification support until court dismissal
  • Partner with public and private agencies to coordinate services
  • Prepare assessments, court reports, and provide testimony
  • Maintain complete and accurate case documentation
  • Prepare families for permanency through training and home visits
  • Collaborate with Adoption Coordinators to prepare children for adoption
  • Participate in 24/7 on-call rotation and respond to crises
  • Work with Care Management and PAL staff to support placements and older youth
  • Compile and submit monthly reports and data
  • Champion cultural diversity and sensitivity in all interactions
  • Use personal or agency vehicles to transport children safely
