Job description

Summary:

Case Coordinators are responsible for managing and coordinating care for clients and families in residential treatment. This includes individual and family case management services to enhance engagement in treatment, facilitating educational and skill development groups in collaboration with the clinical team, and overseeing admission, aftercare/discharge planning, and coordination with external providers.

Shift: Monday - Friday 8:00am - 4:00pm

Pay: $15.50 - $17/hr

Responsibilities:

  • Coordinate client engagement through managing appointments, aftercare plans, schedules, and meetings in collaboration with the treatment team.
  • Maintain communication with referral sources and outside providers through professional interactions, assessing needs, and documenting contacts from admission throughout treatment.
  • Provide case coordination services including organizing transportation and individual case coordination sessions.
  • Assist in the development of aftercare plans, facilitating referrals, and addressing external provider needs.
  • Support clinical services by assisting in group facilitation and supporting client engagement on a weekly basis.
  • Collaborate with the treatment team and maintain professional communication with counselors and team members.
  • Document client, family, and provider contacts in Carelogic according to program timelines.
  • Work with the client navigation team on client aftercare needs and provide onsite support for Enhanced Outpatient Sessions (EOS).
  • Participate in mandatory corporate and program training sessions and engage in professional development opportunities.
  • Perform other duties as assigned. 

Qualifications:

  • High School Diploma or equivalent required.
  • One year of experience in a drug and alcohol or mental health setting is preferred.
  • Driver's License is preferred.
  • CPR/BLS certification is preferred.
