Description

American Traveler is hiring an experienced RN Case Manager for an acute-care hospital assignment requiring EPIC experience and a compact or OK RN license.

Details
  • Acute-care hospital Case Management department
  • Manages complex hospitalized patients requiring care coordination and discharge planning
  • Day shift schedule of four 10-hour shifts per week
  • Rotating weekend schedule required
  • Floating required to all facility campuses within a 75-mile radius
Requirements
  • Active RN license (Standard or Compact)
  • Minimum 2 years of acute-care hospital Case Management experience required
  • EPIC EMR experience required
  • Experience with MCG or InterQual preferred
  • CCM certification preferred but not required
  • Strong clinical assessment, care coordination, discharge planning, and communication skills
  • Ability to work independently within a multidisciplinary care team
Additional Information
  • Coordinate services and facilitate timely transitions of care for hospitalized patients
  • Document accurately in the electronic medical record
  • No blocked scheduling requests allowed
  • Maximum RTO of 5 days per 13-week assignment with no holidays or blackout dates permitted
  • Must reside outside a 75-mile radius of the facility to be considered
  • Local candidates not accepted unless outside the 75-mile radius
  • Former permanent staff must be separated from the facility for at least 1 year to be eligible for travel assignments
