Job description

We are seeking a compassionate and detail-oriented Registered Nurse (RN) for a part-time assessor role focused on in-home patient care assessments and supervisory visits. This position is ideal for an experienced RN who enjoys autonomy, flexible scheduling, and working directly in the community.

Position Overview

The RN Assessor will be responsible for conducting in-home clinical assessments, performing supervisory visits for home care staff, and updating individualized patient care plans to ensure quality, compliance, and continuity of care.

Key Responsibilities
  • Conduct comprehensive in-home patient assessments

  • Perform supervisory visits for home health aides and care staff

  • Review, evaluate, and update patient care plans

  • Document clinical findings accurately and in a timely manner

  • Ensure care plans meet state and agency regulatory requirements

  • Communicate effectively with interdisciplinary care teams, patients, and families

  • Identify changes in patient condition and escalate concerns appropriately

Qualifications
  • Active and unrestricted Registered Nurse (RN) license
