LPN - Wellness Nurse - Assisted Living Community - or RN - Registered Nurse

Since 1960, D’Youville Life & Wellness Community has provided Greater Lowell and surrounding communities with Faith-Based, Nonprofit, Short-Term Rehabilitation, Skilled Geriatric Care and Assisted Living Facility earning the respect of its peers and the trust of the community at large.

We are looking for a Wellness Nurse - Assisted Living Community - RN - Registered Nurse or LPN - Licensed Practical Nurse to join our dynamic team.
  • Part-Time and Per Diem Shifts Available - MUST BE ABLE TO WORK: Every Other Weekend.
  • Pay Range: $33 to $39.60 base pay based on license and experience. Shift Differential added to evening shift.

Wellness Nurse - Assisted Living Community - RN or LPN Qualifications:

  • MA RN - Registered Nurse or LPN - Licensed Practical Nurse License.
  • 1 year of Assisted Living Facility experience preferred.

Wellness Nurse - Assisted Living Community - RN or LPN Job Summary:

  • Implement and coordinate resident care in collaboration with physician and nursing staff.
  • Document all resident care information.
  • Provide functional direction and assistance to nursing support staff.
  • Complete new staff orientation as assigned.
  • Direct resident care to include making round, answering call lights, assist with medications.
  • Complete monthly/quarterly audits, assessments and documentation.
  • Communicate effectively with the residents, families, visitors, members of the health care team, and other disciplines.
  • Participate in screening potential admissions, as needed.
