Description
Surfpoint Recovery is looking for a Discharge Planner we grow and expand. Surfpoint Recovery is a cutting edge substance-use-disorder (SUD) treatment center that has partnered with DailyPay for same day access to funds! Employees no longer have to wait until pay day to get their earnings. We are located in Coney Island, Brooklyn NY. We are an OASAS-licensed and CARF-certified Center of Excellence. Surfpoint Recovery offers inpatient detox, withdrawal, and stabilization services, as well as inpatient rehabilitation services for individuals with substance use disorder.
A Discharge Planner is a person who functions as a consultant for the discharge planning process within the facility providing education and support to clients in the development and implementation of discharge plans for their next level of care.
Schedule: Monday - Friday (9:00am - 5:00pm)
Responsibilities:
  • Effectively provides clinical and psychosocial services to patients, involving patients' support systems including family, friends, counselors, and treatment providers.
  • Dependably coordinates and facilitates the development of individualized treatment plans and actively participates in interdisciplinary treatment team meetings.
  • Provides thorough and accurate discharge planning information for initial treatment plans during interdisciplinary treatment team meetings.
  • Routinely and professionally collaborates with external agencies and providers of service in such areas as mental health and substance abuse to implement an effective, safe and individualized discharge plan.
  • Coordinates and facilitates safe and appropriate discharge plans for patients which include case management referrals and follow-up with outpatient appointments addressing patients identified needs.
  • Routinely provides psychosocial education to patients and families as scheduled and as indicated in treatment plans.
  • Responsible for coordinating referral follow-up and coordinating with post discharge referrals to confirm patients' next level of care.
  • Performs functions related to effectively and efficiently assisting patients through smooth, safe, and quality transitions for aftercare.
  • Establishes and maintains relationships with community providers, referral sources, hospitals, and social support.
  • Ability to research and create linkages based on patient needs.
  • Effectively audits and insures the implementation of Individualized Treatment Plans, Psychosocial Histories and Integrated Summaries within established time frames.
  • Assist with therapeutic programming around group engagement.
  • Routinely demonstrates a sound understanding of and commitment to complying with OASAS program standards.
  • Consistently displays cultural competence and sensitivity and ensures services provided incorporate patient-specific cultural values.

Requirements
MINIMUM QUALIFICATIONS:
  • CASAC (or similar), with Case Management experience
  • Bachelor's degree required
  • Two (2) years of related work experience
  • Bilingual Spanish a plus
