Case Manager – Hickory Treatment Centers

Department: Clinical

Direct Report: Clinical Manager


About the Role

Under the immediate supervision of the Clinical Manager, the Case Manager is responsible for each component of the client’s treatment planning, client’s behavioral health plan, including all programmatic implementation.

Case managers will also coordinate aftercare plans such as MAT service appointments, mental health continuums, primary physician appointments, relapse prevention support within the community including NA/AA meetings and identify transportation for client at time of discharge. The Case Managers will also meet with their assigned case load individually every 7 days to adhere to documentation requirements through Medicaid standards.

Job Qualifications
  • High School diploma preferred,
  • Minimum of 1-2 years of related experience or training
  • Ability to read, write, and comprehend the English language- Simple instructions, short correspondence, & memos
  • Positive demeanor and clear understanding of professional boundaries with clients
  • Ability to follow directives explicitly, adhere to facility policies, provide patient-driven customer service, and exercise time management skills.
  • Dependability, punctuality, accuracy, attention to detail, & thoroughness are all essential to this role.
  • Completion of CPR and CPI before first shift on the floor
  • Schedule flexibility, holiday, and weekend rotation is REQUIRED.
  • Must hold a valid driver's license with reliable transportation.


Key Responsibilities

Client Support & Case Management

  • Provide discharge planning support for all assigned clients
  • Assist clients with housing applications, placement, and coordination
  • Provide employment assistance, including job search support and resource referrals
  • Assist clients with benefit applications (e.g, food stamps, insurance)
  • Coordinate transportation needs for clients
  • Develop individualized relapse prevention plans
  • Complete admissions/intake paperwork
  • Demonstrate cultural sensitivity and ethical treatment of all clients
  • Maintain professional boundaries, appearance, demeanor and communication

Documentation, EMR & Compliance

  • Maintain strict adherence to HIPAA compliance in all communication regarding clients
  • Complete SNAP and BAM documentation as required
  • Update BHP weekly
  • Complete weekly progress notes
  • Document all client sessions accurately and promptly
  • Ensure full compliance with EMR usage and documentation standards.
  • Collaborate with the Primary Therapist to ensure cohesive treatment planning

Professional Conduct, Training & Team Participation

  • Participate in monthly staff training
  • Maintain effective communication and collaboration with the treatment team
