Your future role at a glance

Location: Indianapolis, IN

Facility: Ascension St. Vincent

Department/Specialty: Inpatient Case Management

Schedule: Every Saturday and Sunday, 7:00AM-5:30PM

How you'll make an impact in this role
  • Lead the interdisciplinary care strategy from admission to discharge to ensure every patient experiences a seamless, safe, and well-coordinated transition home or to their next level of care.
  • Advocate for patient needs and personal values by removing financial or logistical barriers, ensuring they receive the necessary resources and support for a successful recovery.
  • Evaluate medical necessity and care levels continuously to ensure patients receive the right treatment at the right time while optimizing hospital and insurance resources.
  • Collaborate with families, clinical teams, and insurance providers to build a unified plan that prioritizes high quality outcomes and prevents gaps in post discharge care.
What minimum qualifications you'll need

Licensure / Certification / Registration:

  • Registered Nurse obtained prior to hire date or job transfer date required.
  • Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted.

Education:

  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
What additional requirements you'll need
  • 1 year of recent acute inpatient Case Management experience.
  • At least 2 years acute care inpatient nursing experience required.
