Concordia at Villa St. Joseph is seeking a compassionate, organized, and detail-oriented Part-Time Case Manager - Skilled Nursing to join our interdisciplinary team. This position plays a vital role in ensuring our short-term rehabilitation residents receive coordinated, high-quality care while supporting successful transitions throughout their stay.

Position Summary for the Part-Time Case Manager - Skilled Nursing:
The Part-Time Case Manager is responsible for coordinating care for skilled nursing short-term rehabilitation residents. This role works closely with nursing, therapy, social work, physicians, and insurance providers to promote positive patient outcomes, efficient care coordination, and timely discharge planning. In addition to case management responsibilities, this position participates in the nursing department's on-call rotation and may be required to work scheduled floor shifts as needed.
Essential Responsibilities of the Part-Time Case Manager - Skilled Nursing:
  • Complete insurance reviews and obtain continued authorization for skilled nursing short-term rehabilitation residents.
  • Coordinate and participate in interdisciplinary care planning meetings.
  • Develop, update, and maintain resident care plans in accordance with regulatory requirements.
  • Collaborate with social work, therapy, nursing, physicians, and other departments to ensure coordinated resident care.
  • Assist with discharge planning and facilitate smooth transitions to home or other levels of care.
  • Order and coordinate delivery of durable medical equipment (DME) and other discharge-related needs.
  • Monitor resident progress and communicate changes with the interdisciplinary team.
  • Maintain accurate documentation in the electronic medical record.
  • Ensure compliance with Medicare, managed care, and facility policies and procedures.
  • Serve as a resource and advocate for residents and their families throughout the rehabilitation process.
  • Participate in the nursing department's on-call rotation, providing support outside of normal business hours as scheduled.
  • Work scheduled nursing floor shifts as needed to support resident care and staffing needs.

Qualifications for the Part-Time Case Manager - Skilled Nursing:
  • Current nursing license required.
  • Previous experience in skilled nursing, rehabilitation, case management, utilization review, or care coordination preferred.
  • Knowledge of Medicare, managed care, and insurance authorization processes is highly desirable.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication, organization, and time management skills.
  • Ability to work collaboratively as part of an interdisciplinary team.
  • Proficiency with electronic medical records and computer documentation.
