Job description


Position Name: RN Manager, Case Management

Location: El Paso, TX

Salary: Up to $130,000 + Relocation Assistance


Overview:

An established, full-service acute-care hospital in El Paso, TX is seeking a full-time RN Manager of Case Management. This facility is a 300+ bed Level II Trauma Center offering comprehensive services including emergency care, cardiac care, women’s services, Level III NICU, and more. The role operates on a daytime schedule with rotating weekends and will oversee a team of 18 FTEs, including Case Managers, Social Workers, and one coordinator. A supervisor will report directly to the Manager.


Job Summary:

The Case Management Manager plays a key leadership role in ensuring coordinated, efficient, and patient-centered care throughout the hospital stay. The role involves working directly with patients and families for comprehensive discharge planning, utilization review, resource management, and collaboration with interdisciplinary teams and third-party payers.


Key Responsibilities:

  • Conduct patient/family assessments for discharge needs and care coordination
  • Perform utilization reviews to ensure efficient resource use and justify continued hospitalization
  • Communicate with physicians and payers regarding treatment plans and level of care
  • Formulate and implement discharge plans including follow-up care, DME, and placements
  • Serve as liaison between patients, care teams, and external providers
  • Supervise and support a diverse team of Case Managers and Social Workers


Qualifications:

  • Active RN license
  • Bachelor’s Degree (Required)
  • Minimum 2+ years of supervisory or management experience in a hospital-based case management setting (Required)
