Social Services Case Manager

Community Health Northwest Florida (CHNWF)

Position Summary

Community Health Northwest Florida (CHNWF) is seeking a compassionate, motivated, and patient-focused Social Services Case Manager to join our integrated behavioral health team. This position plays a critical role in supporting patients whose health outcomes are impacted by social, behavioral, and economic barriers. The Case Manager will provide care coordination, resource navigation, advocacy, and psychosocial support to underserved and vulnerable populations.

The ideal candidate is passionate about whole-person care and experienced in working with individuals experiencing behavioral health challenges, substance use disorders, homelessness, incarceration reentry, and other complex social needs.


Key Responsibilities
  • Conduct comprehensive psychosocial assessments to identify social determinants of health and behavioral health needs.
  • Develop and implement individualized care plans in collaboration with patients, primary care providers, and behavioral health clinicians.
  • Coordinate referrals and follow-up services for:
    • Mental health treatment
    • Substance use services
    • Housing resources
    • Food assistance
    • Transportation services
    • Community support programs
  • Serve as a liaison between patients, families, healthcare providers, and community organizations.
  • Monitor patient progress and provide ongoing support and follow-up.
  • Advocate for patient needs within healthcare and community systems.
  • Participate in interdisciplinary team meetings and integrated care planning.
  • Maintain accurate and timely documentation in the Electronic Health Record (EHR) using SOAP notes.
  • Support patient engagement, appointment adherence, and retention in care.
  • Participate in quality improvement initiatives related to behavioral health and population health outcomes.
  • Perform additional duties as assigned to support organizational operations.

Minimum Qualifications
  • Minimum of 1–2 years of experience in case management or care coordination.
  • Experience working with underserved or high-risk populations.
  • Strong communication, organizational, and problem-solving skills.
  • Ability to work collaboratively within an interdisciplinary healthcare environment.
  • Experience documenting in Electronic Health Record (EHR) systems.

Preferred Qualifications
  • Bachelor’s degree in Social Work, Psychology, Counseling, Public Health, or related field.
  • Experience in a behavioral health, substance use treatment, community health, or FQHC setting.
  • Knowledge of community resources and social service systems.
  • Familiarity with integrated behavioral health models and trauma-informed care.
