Job Description

Staffing/Service Coordinator Home Care Bilingual English/Spanish

Our Coordinators are a liaison between our Home Health Aides, Patient/Client and the RN-Home Care Supervisor. This is a full time onsite position in our Midtown Manhattan location.

As a Service Coordinator you will:

Using Care connect and HHA Exchange identify and assign staff to meet client needs
Provide general information to the public regarding home care services
Collect initial intake information from clients
Communicate with RN-Home Care Supervisor regarding all new clients, changes in staff assignments and client needs
Ensure employee files are complete and up to date with all required documentation
Be available on a rotating schedule for Remote On Call evening and weekend service

Successful Candidates will have the following skills and experience:

One year of Home Care Coordination Experience
Bilingual English/Spanish verbal and written skill is required
HHA Exchange and Care Connect experience is a Plus
Strong verbal and written communication skills
Basic data entry skills
