Job Description
Are you a compassionate and experienced memory care director? Fieldstone at Chester Springs – the area’s newest senior living community opening this Summer - is seeking a Program Director to help build a team of dedicated caregivers and activities assistants that are committed to providing an exceptional life for residents in our Compass Memory Care Neighborhood.

As Program Director, you will be responsible for implementing New Directions ™, Vantage Point Retirement Living’s exclusive memory care program. New Directions™ focuses on evidence and research-based programming that is backed by an understanding and supportive team. General day to day responsibilities will include…
• Planning, organizing, developing, and leading the overall operation of the Compass (Memory Care) neighborhood in accordance with federal, state, and local laws.
• Acting as the Memory Care community champion in programming and activities.
• Training and educating of staff in the New Directions™ program.
• Designing, scheduling, and facilitating the New Directions™ program incorporating a variety of dementia appropriate activities.
• Partnering with the Recreation and Events Coordinator to ensure a variety of appropriate activities are available throughout the day and evening. Ensuring Personal Care staff are actively involved and engaged in activities.
• Prior to move-in, reviewing the resident’s preferences and needs. Educating Personal Care staff of the same.
• Partnering with Nursing to ensure that the residents’ clinical needs are addressed.
• Attending Care Conferences for residents within the Compass (Memory Care) neighborhood as part of the interdisciplinary team.
• Facilitating the development of the Resident Support Plan and Daily Assignment Sheet to ensure excellence in service delivery, safety, hydration, and resident engagement in programming.
• Assisting with the assessment process to determine level of care.
• Partnering with dining services to ensure meeting individualized resident needs and preferences; participates in the dining experience.
• Understanding resident changes in condition and taking appropriate action.
• Reviewing the 24-hour Report/Daily Log. Following up on pertinent information and resident physical/behavioral changes.
• Establishing a cooperative relationship with the local Alzheimer’s Association chapter or other Alzheimer’s advisory organization.
• Ensuring monthly family support group meetings are planned and facilitated.
• Performing other related duties as assigned by management.

While we support continued learning and enrichment, and celebrate a culture that encourages learning new skills, there are base level requirements for this position that include but are not limited to:
• Bachelor's Degree (BA) from four-year college or university in the humanities (i.e., gerontology, human services, social work, or psychology.) Or one to two years of related experience and/or training, or equivalent combination of education and experience.
• Certificates, licenses, and registrations required: Certified Dementia Practitioner (CDP) preferred.
