Job Title: Family Support Worker

Department: Behavioral Health & Wellness Services

Supervisor: Senior Program Manager for School Centers for Wellness

Location: Prince George's Public Schools - (In-Person)

Classification: Full-time - non-Exempt/Hourly position


Synopsis: The Family Support Worker will develop and maintain relationships with families and partner site staff, including teachers and school administrators at school(s) assigned by the Senior Program Manager. The Family Support Worker is pivotal in providing culturally based case management services to families served in the school-based mental health program at Prince George's County Public Schools. The Family Support Worker maintains healthy & appropriate relationships with students and their families.


Note: This position is funded by a grant from the Maryland Consortium on Coordinated Community Supports or the Strategic Initiative Offices at Prince George’s County Public Schools. Employment continuation depends on the grant's renewal.

Qualifications: Required Education and Experience:

  • Associate degree or equivalent
  • A minimum of Two years of experience working with immigrant families
  • Case Management to underserve communities
  • Knowledge of Latino immigrant and immigrant community and ability to work with all segments of the community
  • Ability to build and maintain network systems within and outside of the school setting

Required Skills/Abilities/Certifications/Licenses:

  • Fluently bilingual (Spanish & English)
  • Valid driver license
  • Ability to work in person in and outside of a school setting.
  • High motivation to work in the school-mental Health field.
  • Ability to work evenings and weekends.
  • Strong ethical core.
  • Detail-oriented, self-starter, highly organized
  • Strong positive aptitude to work independently and as part of a team
  • Leadership, interpersonal, and problem-solving skills.
  • Working knowledge of Microsoft Office, proficient in Excel, database management, data entry, Zoom, and Microsoft Teams

Preferred Education and Experience:

  • An associate's or bachelor's degree in human services and/or a bachelor's degree in human services are preferred.
  • Experience working in school settings
  • Experience working with schools, students, and families
  • Experience working in areas of school-based, mental health, and substance abuse.
  • Experience working with Limited English Proficient Latinos at risk for and/or the LGBTQ community.
  • Knowledge of positive parent engagement curriculums.
  • Ability to work with electronic medical records (EMR), BH Works, and other database management and software.
  • Knowledge and understanding of patient/participant confidentiality issues, HIPAA (Health Insurance Portability and Accountability Act), and FERPA (Family Educational Rights and Privacy Act) compliance regulations.

Duties and Responsibilities:

  • Provide case management services to parents/families with economic and/or social needs such as housing, healthcare, employment assistance, food, clothing, education, and other services.
  • Encourage clients referred by the school system to participate in and make use of available community resources and services such as support groups, food pantries, etc., and directly assist them with applications and processes to access available benefits for which they are eligible.
  • Maintain a caseload in compliance with contract terms and agency expectations as defined by the senior program manager.
  • Function as part of both school and agency-based teams
  • Facilitate, as needed, two-way communication between families and school staff.
  • Acts as a client advocate in school, community, and government agencies.
  • Collaborate with the community school coordinator, therapist, school staff, and community partners to provide comprehensive care to students and their families.
  • Collaborate with school administration and staff on needs of students, families and school community
  • Meets with parents, students, and agency personnel and facilitates communication by establishing and maintaining open lines of communication.
  • Organizes, coordinates, and facilitates a wide variety of meetings, workshops, presentations, group interventions, activities, and events for parents, students, and school staff to develop partnerships between families and schools.
  • In collaboration with the therapist, program coordinator, and school staff, coordinate and manage the program enrollment process.
  • Uses strategies to engage families to build trust and rapport with families.
  • Reports incidents of child abuse and neglect
  • Serve as a liaison and maintain a strong relationship with school administration, school counselors, school phycologists, school nurses, teachers, and other school-related staff.
  • Serve as a liaison between LCDP and program stakeholders for any school-based mental health initiative.
  • Document daily services and provide weekly/monthly written and data reports as required.
  • Accountable for implementing Individual Care Plan and documenting client interactions within 48 hours (about 2 days).
  • Attending required training to increase/maintain skills.
  • Attend program, department, and all staff meetings and all mandatory La Clinica activities.
  • Follow La Clinica's Code of Conduct and all other personnel Policies and Procedures.
  • Perform other duties as assigned.

Supervisory Responsibilities:

  • None.

Physical Requirements:

  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift to 30 pounds at times.
  • Must be able to work outside and stand up for a prolonged period during outreach events.
  • Ability to work weekends and evenings as needed.
  • Able to perform duties in different La Clinica locations as required.
