Job Responsibilities:
- Provide basic patient care under direction of nursing staff
- Assist patients with daily activities, such as bathing, dressing, and eating
- Help patients with mobility needs, including transferring them from beds to wheelchairs, and vice versa
- Measure and record patients' vital signs, such as blood pressure, temperature, pulse, and respiration
- Observe patients' conditions and report any changes to the registered nurse or healthcare provider
- Maintain a clean and safe environment for patients by sanitizing surfaces, cleaning rooms, and organizing supplies
- Assist with toileting needs, including helping patients use bedpans, commodes, or toilets
- Provide patients with emotional support and companionship
- Follow all healthcare protocols and standards to ensure patient safety and quality care
- Document and report activities and patient observations
- Assist with the setup and administration of meals, ensuring dietary restrictions are followed
- Perform light housekeeping tasks related to the patient's immediate environment
- Replenish patient care supplies and manage inventory to ensure necessary items are in stock
- Encourage and assist patients with exercises and other rehabilitative measures as per care plan
- Support patients by providing housekeeping and laundry services; shopping for food and other household requirements
- Prepare and serve meals and snacks
- Active certification as a Certified Nursing Assistant (CNA) or Home Health Aide (HHA)
- Completion of a state-approved CNA or HHA training program
- Ability to pass a criminal background check and drug screening
- Current CPR and First Aid certification
- Physical ability to perform job-related duties, which may require lifting, bending, standing for long periods, and providing mobility assistance
- A minimum of one year of experience in home health care or in a long-term care facility preferred
- Excellent communication and interpersonal skills - Proficient in documenting and reporting patient status and care provided
- Knowledgeable about patient hygiene, nutritional needs, and vital sign monitoring
- Capable of working independently or as part of a team
- Valid driver’s license and reliable transportation
- Flexibility to work various shifts, including evenings, weekends, and holidays
- Demonstrated ability to follow written and verbal instructions effectively
- Strong observation skills and attention to detail
- Compassionate and patient-focused approach to care
- Familiarity with standard concepts, practices, and procedures within the home healthcare field
- Willingness to continue education and training as required by state regulations or employer policies
