Care Lync Georgia is a Social Services agency focused on providing Service Coordination (Supports Coordination/Case Management) services to adults diagnosed with Intellectual, Developmental and Physical Disabilities and Autism Spectrum Disorder throughout Georgia who are enrolled in the Department of Human Services, Medicaid Waiver Program. Our Service Coordinators work with adults to help them to navigate, understand, and secure the services needed in order to live independently in their home and community.

General Summary

The Care Coordinator (LPN) provides case management activities necessary to meet the needs of clients assigned to Care Lync Georgia. This position is responsible for the initial assessment determining Level of Care, appropriateness of HCBS for potential consumers, development of individualized care management plans; implementation of the care plan, reassessing consumer needs in response to condition changes; and the monitoring and evaluation of all clinical outcomes to ensure that services are provided in a professional, comprehensive and cost-effective manner.

Essential Duties and Responsibilities

Responsible for completing comprehensive assessments that either confirm or deny nursing home level or care per the EDWP Case Management Manual.

Under direction and supervision of a licensed, professional registered nurse, performs medical and social assessment of nursing facility intermediate level of care and appropriateness for community-based services for Medicaid recipients.

Completes assessments on clients following appropriate guidelines and submits all assessments to a registered nurse for review and approval. LPN makes corrections and additions as recommended by supervising RN. Under supervision of a licensed professional registered nurse, the LPN functions as a member of an interdisciplinary team including client's physician and serves large geographic areas which may include parts of one large county and/or many small counties which involve extensive travel.

Qualifications

Graduate of an accredited diploma or baccalaureate school of nursing

Two years of experience in case management in a health care field. Preference given to national certification in case management.

Experience with the CCSP and Source Waiver programs is a plus.

Experience with Georgia Department of Family and Child Services (DFCS) is a plus.

Active license within the state of Georgia

Own reliable transportation

Must be able to function independently and effectively in a community-based environment.

Proven interpersonal skills with ability to communicate effectively.

Proven decision-making skills, organizational, self-disciplined, and time management skills.

Must be familiar with general use and functions of the computer, such as, usernames and password concepts; internet; e-mail; navigation of computer desktop or laptop, including starting programs, using files, and windows, effectively use navigation buttons and tool bars.

Assessment Duties

Schedules appointment for face-to-face interview with a client at his or her residence, hospital, long-term care facility, or other appropriate site as indicated.
    • Collects medical documentation which supports a nursing home level need.
    • Analyzes and interprets all medical, social information as compiled, and obtains additional information as needed: e.g., consultation with RN supervisor, physician and other professionals.
    • Appointments for Initial Assessments must be scheduled within 5 business days from assignment.

Conducts assessments in a face-to-face interview with the client and/or authorizes representative using the intermediate level of care criteria and the MDS-HC.
    • Receives client signatures for required forms.
    • Develops the Comprehensive Care Plan to reduce the risks identified in the MDSHC triggers.

Completes a comprehensive note which summarizes assessments, forms reviewed and notes any changes to be reviewed with the assigned Case Manager. Collaborates with RN re ECM/TCM changes that may influence eligibility.

Saves all client documents and workflow updates to Care Lync operated website and share point.

Submits completed assessments to the registered nurse supervisor for review and recommendations of approval or denial. LPN completes any recommendation corrections prior to submitting the assessment packet to the RN for approval/denial review and applicable submission to Alliant Health Solutions (AHS) and the client's physician.

All Completed Assessments must include at minimum:

Medical documents requested/received from client- Acceptable medical documentation lists medications confirming diagnoses or lists diagnoses to justify diagnosis and Nursing Home Level of Care.

Administrative Duties

Assists with statistical data compilation within Care Lync operated website and share point.

Consults with Management Team for any issues with scheduling clients.

Attends organizational meetings and training as required.

Keeps supervisor informed of progress and problems in weekly supervisions.

Submits Invoices bi-weekly for completed assessments.

Working Conditions

While performing the duties of the job, the employee is regularly in an office type or residential (home) setting with varying degrees of background noise.

Physical Requirements

While performing the duties of this job, the employee may be required to stand, walk, sit, reach, handle, to feel objects (such as tools or controls), climb or balance, stoop, kneel, crouch or crawl. The employee must have the ability to read and interpret documents, write, and communicate effectively. Specific vision abilities or also required by this job include close vision, distance vision and peripheral vision. The employee must regularly lift and/or carry up to 50 pounds. The employee must have the ability to push/pull up to 25 pounds. Must be able to maneuver stairs often while carrying objects.
