Job Description

 

I.                  Position Title: Supervisory Nurse Care Coordinator

II.                 Status: Hourly

III.                Evaluation:  90 Day Introductory Period with evaluation verbal or written

IV.                Duties/Responsibilities:

  Reports to Administrator/Executive Director and is responsible to provide the following in the absence of the Administrator.

     i.      Responsible for the daily operation of the agency which includes private duty services; hourly and visits; and the staffing of all health care professionals of the agency.

     ii.      Act as a liaison between management and care staff.

     iii.      Responsible for overseeing recruitment, selection and evaluation of all care employees within the agency in accordance to job descriptions.

     iv.      Responsible for establishing standards of performance and enforcing adherence to these guidelines by the agency care staff

     v.      Responsible for coordinating and overseeing client care and conduct client satisfaction calls and follow up

     vi.      Responsible for coordinating and supervising orientation and continued education to agency care staff. 

     vii.      Responsible for adherence of care staff to policies and procedures of agency

     viii.      Responsible for adherence to all federal and state mandates that refer to home and community support agency and compliance thereof.

     ix.      Responsible for the accuracy of all public accessible information and activities of the agency

     x.      Must be able to conduct service including but not limited to coordinating scheduling, dealing with issues related to services, and take intakes after hours according to appropriate rotation.  This includes weekend on-call and weekly on-call.

     xi.      Must be able to meet expectations of coordination of care for services hired by families of agency

     xii.      Must be able to deal appropriately with care employee issues along with holding those working for agency accountable to policies

     xiii.      Ensure that each client admitted to services receives, in writing, the patient bill of rights listed in K.A.R. 28-51-111.

V.                Supervises supportive services in accordance with a plan of care in a client’s home. Has the responsibility for the observation, assessment, counsel, scheduling care, and health teaching of the ill, injured, or confined, and the maintenance of health and prevention of illness of others.

VI.              Records the health needs of the clients and work with other medical professionals to provide a safe supportive service to client.

VII.           Performs and completes start-of-care, recertifications, resumption-of-care, and discharge comprehensive assessment and associated paperwork in accordance to agency policy.

VIII.        Initiates or delegate the creation of a Plan of Care, revises as necessary.

IX.              Evaluates the effects of the care given and regularly reevaluates the client’s needs.

X.                Makes referrals to other agencies or services as needed.

XI.              Submission of documentation per agency policy.

XII.           Participates in in-service programs and staff meetings.

XIII.        Directs the activities of the supportive care workers

XIV.        Complies with Home Care Agency Compliance Plan.

XV.           Complies with Home Care Agency’s HIPAA Policies.

 

XVI.        Qualifications:

 a.       Experience:   Understanding of budgets and financial statements; human resources; operational oversight; marketing; and or community service within a home health agency.

 b.      Special Skills:  Strong management, communicative skills, and computer proficiency. Meets at least one of the following:

     i.      Minimum experience of 1 year in healthcare or healthcare management

     ii.      Is a qualified health professional

 c.       Must be able to work independently and within a team. 

 d.      Must have an active nursing license in the state.

 e.       Must pass a criminal history check as well as the National Sex Offenders database check.

 f.        Physical/Psychological Demands:

     i.      Visual and auditory acuity

     ii.      Ability to lift, stoop, push, and pull.

     iii.      Must be able to cope with death and/or severe diseases/disorders and the families who suffer them.

XVII.     Special Requirements:

 a.       Must be able to handle stress when dealing with families personal issues.

 b.      Must participate in coordination of care activities.

XVIII.   Reporting:

 a.       Reports directly to Administrator or Executive Director of the agency

 b.      Reports to Briggs Management and Governing Board.
