North Suffolk is looking for a Clinical Coordinator to join our Intensive Community Wrap-Around Services Team! Under the supervision of the Program Director, the Clinical Coordinator leads the team in the assessment, treatment planning and service delivery processes and is primarily in home and community settings providing coaching and working side by side with persons served. The Clinical Coordinator oversees the development of the persons’ treatment plan and subsequent reviews and revisions to ensure the goals, objectives and interventions reflect the Clinical Formulation-Interpretive Summary and the prioritized assessed needs. Services are provided based on Positive Behavior Supports and Recovery tenets and are aimed to promote overall health and well-being of persons served.


What you’ll do:
  • Engage people and partner with them to improve their health and wellness, achieve symptom stabilization and self-management, develop and/or restore skills impacted by mental illness and live a self-directed life.
  • Assist people with autism spectrum disorder, psychiatric and substance use disorders to identify personal priorities, preferences, strengths, and interests in order to help them establish goals that support a life in recovery.
  • Provide clinical services in timely and flexible ways that are consistent with the principles and practices of trauma informed care and person centered approaches.
  • Carries out the assigned interventions and action steps of the treatment plan, through the provision of direct support and rehabilitation services to people on an individual, group, and family basis in the office and in community settings to teach behavioral symptom-management techniques to alleviate and manage symptoms not reduced by medication and to promote personal growth and development by assisting people to adapt to and cope with internal and external stresses.
  • Organize and lead individual and group social and recreational activities to structure each persons time, increase social experiences, and provide opportunities to practice social skills and receive feedback and support.
  • Document each person’s progress in the confidential record according to established methods and procedures.
  • Other duties as assigned by the Program Director.


What you’ll need:
  • Education/Certification:
    • LPHA licensure required: LICSW, LCSW, LMHC, LRC, LADC1, M.D., R.N., Registered Occupational Therapist or licensed Psychologist.
  • Experience:
    • Previous experience in the provision of clinical or rehabilitative services to adults with chronic and persistent mental health issues and/or substance use disorders preferred.
    • Previous supervisory experience preferred, but not required.
  • Other requirements:
    • Must have valid Driver’s license. Must be MAP certified, CPR/first aide certified within five (5) continuous months of employment/date of hire.
