Medicaid Care Coordinator

Temp-to-hire Medicaid Care Coordinator, Chicago, IL

Our client, an ACO, located on the near west side of Chicago, is seeking to add to their team another Medicaid Care Coordinator on a temp-to-hire, full time basis.  This position is a hybrid position with three days in office and two days at home.  The Care Coordinator is part of three-person team that is involved with managing the Coordination of Care for patients that are enrolled in the County Care Medicaid Program.

Duties of Care Coordinator-Navigator:

• Complete Monthly Health Risk Assessments (HRAs) on members
• Outreach to patients who have been in the ER or Observation Stays
• Schedule follow-up appointments for patients who have been discharged from ER or observation stays
• Outreach monthly & complete HRSs for patients who have a depression screening of low & medium
• Outreach monthly to maternity patients to ensure they are completing prenatal & post-partum visits
• Address any Social Determinates of Health disparities members may have.
• Documentation is done in MHNConnect Portal.

Position Requirements/Knowledge & Skills

• High School diploma or equivalency, associate degree preferred
• Highly proficient with use of computers and must be proficient in MS Office products especially Excel
• Minimum of 2 years’ experience in healthcare
• High degree of creativity & problem-solving
• Ability and patience to navigate and impact complex systems of care
• Ability to develop & employ effective customer relationships w/patients and healthcare team members
• Ability to work independently & also as a team with a wide variety of individuals.
• Bilingual Spanish/English is a plus, but not a requirement.
