NOW HIRIN: Social Worker

We’re seeking a compassionate and motivated Social Worker to join our dedicated team. In this role, you’ll play a key part in supporting the emotional and psychosocial well-being of our residents, working closely with families, interdisciplinary team members, and community partners.

Social Worker Responsibilities:
  • Assist residents and families in adjusting to short-term rehab or long-term care placement

  • Conduct psychosocial assessments and care planning

  • Coordinate discharge planning and referrals

  • Advocate for residents’ rights and help resolve concerns

  • Participate in care plan meetings and interdisciplinary team discussions

  • Maintain accurate documentation in compliance with state and federal regulations
